The death certification process varies from state to state, despite the fact that most states have an electronic registry. All states, however, use the same form, the United States Standardized Certificate of Death. There are three parts to the certificate. Because certification must be completed within several days of death, additional checkboxes for deaths include “could not be determined” and “pending investigation.” Individuals with formal medical training and those without are both eligible to complete this section, according to state rules. When new information becomes available, certificates can be modified, though the process varies by state.
