How to get a copy of a birth certificate

Miscarriage occurs in one out of every four pregnancies, and the pain of that loss is as real as life itself. Creating a culture of life means paying tribute to the lives of those who have died as a result of miscarriage, as well as the bereaved families.
Women and families in Nebraska can now choose to receive state-issued commemorative birth certificates for babies lost to miscarriage, regardless of gestational age, thanks to a special, life-affirming bill passed into law by the Nebraska Legislature in 2018.
The law creating this type of certificate in Nebraska is only the second of its kind in the country. Nebraska is the first state to offer retroactive certificates, allowing women who had a miscarriage prior to the law’s passage to receive one. There is no minimum gestation period required to receive a certificate.
Have a licensed health care practitioner (your doctor, a nurse, an OB/GYN, or their designee) sign the physician worksheet certifying the miscarriage. The doctors worksheet can be found here.
