Follow the signs for the Labor and Delivery entrance at St. Joseph’s Hospital and Medical Center, located at 350 W. Thomas Road, Phoenix, AZ 85012. We will gladly assist you in obtaining copies of your medical records. You must first download and correctly complete an authorization form in order to request health records. We will release your medical records once we receive your completed form.
A guardian or court-appointed personal representative may sign on behalf of the patient if the patient is unable to sign the request form due to a physical or mental disability. If the patient does not have a guardian or personal representative, the person who has medical power of attorney may sign on his or her behalf.
