Follow the signs for the Labor and Delivery entrance at St. Joseph’s Hospital and Medical Center, located at 350 W. Thomas Road, Phoenix, AZ 85012. When you arrive, the admitting staff will arrange for you to be taken to the Family Birth Center’s triage. You’ll get an initial assessment in triage, which may include fetal monitoring to see if you’re in labor. Depending on your condition, you may be admitted to the hospital for delivery or given instructions to return once your labor has progressed.

We will gladly assist you in obtaining copies of your medical records. You must first download and correctly complete an authorization form in order to request health records. We will release your medical records once we receive your completed form.
A guardian or court-appointed personal representative may sign on behalf of the patient if the patient is unable to sign the request form due to a physical or mental disability. If the patient does not have a guardian or personal representative, the person who has medical power of attorney may sign on his or her behalf.
We can provide medical records and information, but we are unable to provide copies of birth certificates. Please contact the Colorado Department of Public Health and Environment at 303-692-2200 for a copy of your birth certificate.

We will need your written authorization and a legible copy of your valid photo identification if you want us to send a copy of your medical record to another healthcare facility/provider, insurance company, attorney, or another individual.
If we send records directly to another healthcare provider, there is no charge. There may be a fee for copies of medical records that you pick up or that are mailed to you directly. The fee is $15.00 for 1-30 pages and $.50 per extra page over 30 pages, as set by the State of New Hampshire.
