Form DE 2501, Claim for Disability Insurance (DI) Benefits, is a mail-in form that allows eligible workers to request worker-funded benefits for a full or partial loss of wages due to non-work-related disabilities.
If you file a disability claim, your employer will be informed, but your personal information will be kept private. According to the California Unemployment Insurance Code (CUIC), a disability is defined as any physical or mental illness or injury that prevents you from working regularly. Surgical procedures and complications related to pregnancy or childbirth are also included.
If you are covered by a voluntary plan or filing for non-industrial disabilities insurance benefits, you cannot use Form DE 2501. You must be unable to do normal work for at least 8 days, be employed or actively looking for work at the time of the disability, have lost wages as a result of the disability, and/or be under the care or treatment of a licensed physician in order to be eligible. Claimants must complete the form and provide medical certification of their disability within 49 days of becoming disabled.

Short-term Disability Insurance (DI) salary replacement benefits are provided to eligible employees through the State Disability Insurance (SDI) program. If you are unable to work and are losing wages due to your own pregnancy, childbirth, or non-work-related illness or injury, you may be eligible.
The day you became unable to work as a result of your disability marks the start of your disability. After you’ve established a valid claim, you can’t change the start date or alter the base time. Before filing your claim, call DI at 1-800-480-3287 if you have any questions about your claim start date.
If you submit your claim more than nine days after the first day of your disability, but no more than 49 days after the first day of your disability, it will be considered a late claim, and you will lose benefits.
Your physician/practitioner must certify your disability by filling out and submitting the medical certification section of the paper claim form, either using SDI Online or the DE 2501 Part B – Physician/Certificate. Practitioner’s You must have your physician/practitioner complete and sign the form and send it to the EDD within 49 days of the start of your disability or your benefits will be terminated.
