Carry all original supporting documents with you to the Consular Officer’s office. These papers may be photocopied and attached to the application. Make sure these papers’ photocopies are legible.
Fee: A fee of US$25.00 per copy, plus an extra fee of US$2.00 (two only) per application to the Indian Community Welfare Fund (ICWF). The fees can be paid in cash or by two separate money orders/cashiers’ checks drawn in favor of the Embassy of India, Washington DC, for the fee and the ICWF.

If you are applying for a passport for the first time or replacing a lost passport, you must present your original Philippine Statistics Authority Birth Certificate on Security Paper (PSA). PSA Birth Certificates may also be required for a minor passport applicant as proof of filiation (i.e., evidence of a child’s connection to his or her parents).
You must report your passport as lost and obtain a police report at the closest police station. When applying for a replacement passport, you will also need to complete an Affidavit of Loss and present your original PSA Birth Certificate.
Please send your old passport to the Embassy for cancellation via postal mail. We will then send your new passport, along with your cancelled old passport, via registered mail. Normally, registered mail takes 3 to 4 days to arrive in Norway, and 4 to 5 days to arrive in other Nordic countries.

If you do not have an original state-issued birth certificate, you can submit a combination of early public records with your passport application and identification as proof of your U.S. citizenship. A birth certificate or a Letter of No Record must accompany early public records (detailed below).
You can submit a Delayed U.S. Birth Certificate if you were born in the United States but do not have a birth certificate because your U.S. Birth Certificate was not filed within the first year of your birth. What type of birth certificate is needed for a passport

Originals of all supporting documents to be delivered to the Consular Officer should be carried (photocopies of which are to be attached with the application). Bring a notarized copy of the documents attached to the application if you are unable to present the original documents at the counter. If the notarized copy is not legible, the Consulate may request the original. The passport, however, must be delivered in its original form.
Fee: A non-refundable fee of $25 (twenty five dollars) plus a $2 Indian Community Welfare Fee (ICWF) per attestation, effective October 1, 2012. Send a trackable pre-paid return mailing envelope (USPS ‘Express Mail’ or UPS) if you want the attested documents returned by mail (No Fedex).
Fees should be paid by money order or cashier’s check made payable to the “Consulate General of India, San Francisco.”
THE CONSULATE WILL NOT ACCEPT A BLANK CHECK OR MONEY ORDER.
