Birth certificates in Alabama are confidential records with limited access for 125 years after the date of birth, according to state law. They can be purchased by paying the appropriate fee to the following individuals:
A birth certificate search costs $15.00, which requires one certified copy of the birth record or a “Certificate of Failure to Find.” Each additional copy of the same record ordered at the same time would cost $6.00. An extra fee of $15.00 is charged to expedite a request. Payable to “State Board of Health” by check or money order. Please do not put money in the mail. There are no refunds on fees.

After receiving a request from an adoptive parent or an adopted individual 19 years of age or older, the Center for Health Statistics may prepare and register a Certificate of Foreign Birth as of January 1, 2013. A child who has automatically obtained United States citizenship as a result of a foreign adoption and has a Certificate of Citizenship in accordance with the Child Citizenship Act may have a Certificate of Foreign Birth placed on file. The Center for Health Statistics will prepare a Certificate of Foreign Birth after receiving the necessary documentation and the appropriate fee. All birth certificates and death certificates issued in Davidson County are processed and stored by the Vital Records Division. Birth certificates are issued after 1950, and death certificates are issued after 1966. Medical and dental records of people who have received treatment at a Metro Health Department clinic are also available from the Vital Records Division.
To receive a certified copy of a birth or death certificate, you must provide identification. A copy of your valid driver’s license is an example of proper identification. Along with your completed application and payment, please send this in.
Please allow seven (7) business days for all mail orders to be processed. A parent, child, or spouse is eligible to receive a certified death certificate with the cause of death. All others will be required to provide valid documentation to back up their claims. A valid government-issued photo ID is required for all requestors. For birth and death records, you can also request certified copies of vital records online through our approved third-party vendor VitalChek or go to your local county office. Before visiting your county office, it is highly advised that you call first.
At the Customer Service Window, you can fill out a paper order form and show proper identification. If you are not a qualified applicant, you must provide evidence of your right to the requested record at the time your application is submitted, or you must have a qualified applicant submit the application on your behalf. As you enter our office, order forms are on the counter to your left. At our customer service window, we accept cash, check, credit/debit cards, and money orders for our fees.
Even if you don’t have identification on you, kiosk orders automatically authenticate your identity. Once your order has been transmitted for processing, you will be asked to provide identification documents at the front counter if you do not pass the identity verification process. If you are not a qualified applicant, you must provide evidence of your right to the requested record at the time your application is submitted, or you must have a qualified applicant submit the application on your behalf. A $4.00 vendor fee is applied to all orders received through the kiosk. This is a non-refundable fee.
