The Office of Vital Records of the Navajo County Public Health Services District (NCPHSD) provides certified copies of Arizona deaths from February 1, 2008 to the present. You may contact the Arizona Department of Health Services, Office of Vital Records, for a certified copy of any other death that occurred in Arizona. Over the counter or by mail, we approve completed applications. Our office is located at 117 E. Buffalo Street, Holbrook, AZ 86025, and we issue death certificates there. An electronic database in Arizona is used to issue certified death certificates. We can only issue what is stored in the database. Deaths that occurred in the previous 30–90 days may not be completed and entered into the database. We are unable to accept faxed or emailed applications. Along with a completed application, acceptable identification and the appropriate fee must be submitted. Please see the fee schedule below for more information. When the state entered all of the data from original birth certificates from 1950 to 1989 into a computer database for all of the counties to use, they only entered the information that they felt was important at the time. All of the original documents are being scanned into a database so that they can be published electronically by all of the counties.

Cash, traveler’s checks, cashier’s checks, money orders, Visa, and MasterCard are all acceptable forms of payment. Cashier’s checks and money orders must be made payable to the Office of Vital Records and must be for the exact amount.
For authenticated copies of death certificates, same-day service is not available. Applicant’s applications will be approved, and certified copies will be mailed to them.
