Our offices will now provide certified copies of birth certificates dating back to 1930 from all counties in the state of Florida. An eligible recipient, such as the Registrant (must be 18 or older), or Parent, must complete the Birth Certificate application (must be listed on the birth certificate record). At the time of application, valid identification must be submitted. A valid driver’s license or identification card, passport, or military ID card are the most common forms of identification accepted.
