Baby has no birth certificate following dad’s name change
When a child is born to married parents, one or both of them will sign the birth certificate with the child’s name. The original name as well as the new name given at baptism will be included on the full copy certificate. The modified baptismal forename of the child will be the only thing recorded on the short version certificate (s). Parents should fill out a form at the Register Office if their child has not been baptized. This procedure can be carried out at any time after the birth, as long as the change of name occurred within 12 months of the birth registration. The amended name(s) will be inserted into space 17 of the original birth registration once the form is completed. The amended registration can then be bought as certified copies.
