Any person born in the state of Ohio will now have certified copies of their birth certificates from the Toledo Lucas County Health Department. In the past, you had to get your birth certificate from the county where you were born. Statewide Issuance allows you to receive your Birth Certificate without having to travel to your birth county or submit an application by mail. Only our downtown location in Lucas County can now provide you with the certificate.
A certified copy of a birth or death certificate costs $25 per copy. Cash, check, credit card, and money order are all acceptable forms of payment. Purchases made with a credit card incur an extra processing fee. We can no longer provide uncertified copies of birth or death certificates due to changes in state law.
Using a major credit card over the phone: You can now order your Birth Certificates over the phone using a major credit card (Discover, MasterCard or Visa). To place an order, dial 419-213-4100, Option 6. Certificates are typically mailed the same or next business day. Please allow 7-10 days for delivery via USPS. *If you pay with a credit card, there will be an extra fee.

Ordering birth certificate

A company must be at least 51 percent owned and operated by a U.S. citizen(s) and a member(s) of one or more of the following minority groups to be considered for MBE-WBE status: Latino, Hispanic, Black, Asian, Pacific Islander, American Indian, or Alaska Native.
Birth certificates, US passports, US military records, tribal role numbers assigned by the Bureau of Indian Affairs, or other official documentation that states ethnic identification are used to identify race/ethnic background.
Please keep in mind that the review and approval of MBE-WBE certification will take up to 8 weeks. A certificate will be issued once the company has been approved, and it will be listed on our MBE-WBE list by business services offered.

In Person: Go to the Toledo-Lucas County Health Department’s Vital Statistics Office. Your certificate will be processed and released in about 15 minutes in most cases. This service is only available at our Downtown location.
By Mail: Return the completed Death Record Request Form, payment for the certificate, and a self-addressed stamped envelope to Vital Statistics, Toledo-Lucas County Health Department, 635 N. Erie Street, Toledo, OH 43604. Your certificate will be returned back to you within two business days of receiving your request. The return mail will take 7-10 business days. Please include the following information when asking by mail: Name on record, Date of birth, County of birth, Mother’s maiden name, Father’s name.
