Unless the person applying for a minor child’s certificate is a parent, a person applying for a birth or marriage certificate who is not the person named on the certificate must give written permission of the person named on the certificate. The person named on the certificate must date and sign the permission. This written consent must be faxed or mailed to Vital Statistics with the application in order for it to be processed. Without this written consent, certificates will not be issued. A written consent is obviously not possible when applying for a deceased person’s birth or marriage certificate. Please include a copy of your death certificate with your application.

Who Can Submit an Application for a Birth Certificate?

Name of individual (if married, maiden name), date of birth, place of birth, father’s name, father’s place of birth, mother’s full maiden name and given names, mother’s place of birth, connection to person named on certificate, reason for certificate request, daytime phone number, and return address are all required for obtaining a birth certificate. There are two types of birth certificates: short and long.

The information on the index cards is limited. The only information on the Birth and Death index cards is the person’s name and the date of birth or death. The only information on the marriage cards is the groom’s name and the date of the wedding. These cards are organized by year, and within those years, they are sorted by last name in alphabetical order.
