The State Office of Vital Records must receive a certified copy of the Court Order, a $23 fee, and a completed Amendment to Reflect a Court Order Change of Name form. As part of the fee, you will receive a copy of the amended Certificate. Our office or any local California registrar’s office will provide you with an Amendment form.
